Student Medical Plan ID Card
Each student participating in the Student Medical Plan has access to their ID card . To create your ID card enter your student ID number, date of birth and email address assigned to you by your educational institution - case.edu, cim.edu, cia.edu. Once you have printed your ID card, present it to the provider to facilitate prompt payment of your claims.
