Completing Your Financial Aid Application

Below you'll find the most common forms in use by the Office of University Financial Aid. Some of these are required by the office as part of a student's initial application for financial aid. Others are requested by the office on a case-by-case basis as documentation of special circumstances.

All information provided to the Office of University Financial Aid by the student, or by the student's parents or spouse, is completely confidential.

Although there are a large number of forms listed below, only a small number will apply to a given student. Please do not submit all these forms. As a general rule, if you don't know what the form is, or you're not certain it applies to you, please do not submit it.

These forms pertain to the 2025-26 academic year only. If you have any questions regarding the type of forms you should fill out, please do not hesitate to contact us.

Memorandum of Assistance (MOA)

The Memo of Assistance form is required of all graduate and professional students applying for financial aid. Unlike most other forms, it is not completed by the student. Instead, it is completed by the graduate department for the program in which the student is enrolled. A graduate department representative completes the form and forwards it directly to the Office of University Financial Aid, certifying the following information:

• Details of the degree and program
• Exact number of credit hours and tuition fees charged for the academic year
• Type and amount (if any) of graduate financial assistance being received by the student through the department in question

This form is required of all graduate and professional students applying for aid at CWRU, even if said student is not receiving any financial assistance from his or her department. If you are not certain that the form is going to be sent by the university department in question to the Office of University Financial Aid, please contact the department to arrange this.
